Please notE: if authorization is required for surgery, it is the responsibility of the surgeon’s office to obtain & provide it TO GVSC OR THE SURGERY WILL HAVE TO BE RESCHEDULED.

*Patient has to have referral from PCP to surgeon and an authorization for GVSC.

Financial Policy

We feel that communication regarding our financial policy assists us in providing the best service for our patients.  For this reason, we feel it is important to clarify our policies in advance of your procedure. 

Payment estimates for procedures not covered by insurance or third-party payers is expected prior to the time of service. For procedures covered by insurance or third-party payers, we accept assignment of insurance benefits. Co-pays, coinsurance and deductibles are due prior to the time of treatment. This includes patients with Medicare coverage. If there is a secondary coverage involved, be sure to provide us with that information so we may bill appropriately.

Co-pays, coinsurance and deductible amounts are an estimated calculation based on anticipated services at the time the procedure is scheduled.  Adjustments shall be made accordingly after the surgeon has completed the procedure.

Grand Valley Surgical Center is not party to contracts held between the patient or responsible person and the insurance company.  If payment is not made by the insurance carrier within 45 days of filing, the balance will be automatically transferred to patient responsibility. Payment by patient is expected within 10 days of notice of insurance non-payment.  In the event an account is placed with an attorney or collection agency, the responsible party is also billed for collection fees, reasonable attorney’s fees and court costs. Please let us know immediately if you change your address, telephone number, health insurance provider or place of employment. Correct information is vital for appropriate claims and payment submissions.

Anesthesiology services at Grand Valley Surgical Center are provided by an independent physician group, Anesthesia Medical Consultants, P.C. Their services will be billed separately from Grand Valley Surgical Center, L.L.C.  Any questions regarding anesthesia/services billing should be addressed directly with that practice.

Procedures performed by your physician will also be billed separately. Your physician may also order additional laboratory, pathology or radiology tests.  Grand Valley Surgical Center fees do not include these procedures.
